  Online Adjudication Request 

Please complete the personal information boxes below to submit a request for adjudication, reconsideration, file a Motion to Vacate, or appeal to the Traffic Adjudication Appeals Board. You may appeal the denial of a request for reconsideration, a liable decision on a Motion to Vacate, or a liable decision on failure to appear for a hearing. You must submit any documents or evidence that you want the hearing examiner to consider except that no additional documents can be submitted to the Traffic Adjudication Appeals Board. You will not be able to add documents or photographs after the form has been submitted for processing. The technical requirements for the file submittal format are listed at the end of the next page.

Once you submit an adjudication request online, you will receive an email confirmation. If you do not receive an email confirmation, then you have not successfully completed the process and need to try again. Failure to successfully complete the online adjudication request will lead to the ticket doubling and/or being sent to collections.
